The Research Foundation for SUNY is pleased to announce the Caregiver Wellness Specialistposition to assist with the Alzheimer's Disease Caregiver Support Initiative's (ADCSI) caregiverwellness program. This is a full time, in-person position with an office located in Plattsburgh tofocus on caregivers primarily in the Clinton, Essex, and Franklin county region.The goals of the ADCSI initiative are to expand the safety net for caregivers, develop and sustainan innovative array of support services, improve coordination and access to new and existingsupport services and enhance access to respite and health and well-being services tocaregivers.The Caregiver Wellness Specialist works as part of a caregiver support team. Positionresponsibilities include, but may not be limited to, the following:Implement strategies to promote the Caregiver Wellness program along with theservices of the ADCSI primarily within the Northern region (Clinton, Essex and FranklinCounties).-Attend outreach events, educating and promoting ADCSI and Caregiver Wellnessactivities to community agencies.-Attend scheduled wellness activities/events throughout the three-county region with afocus on physical activity/relaxation, cognitive stimulation, diet/nutrition and socialengagement for informal caregivers educating on the importance of health and howwellness plays a role in reducing caregiver stress.-Prepare for and attend tabling events throughout the three-county region to promoteoutreach to professionals and informal caregivers.-Attend scheduled memory cafes, allowing for increased communication andconnections with caregivers in rural and underserved communities.-Collaborate with Regional Care Navigators to determine needs and to promoteparticipation in Caregiver Wellness events/activities.- Following a referral from the Care Navigators, will meet with caregivers interested inMusic and Memory, assisting with the development of a playlist tailored to the likes ofthe care receiver.-Maintain the Music and Memory tracking system to ensure the return of equipmentwhen no longer in use.-Complete and maintain required quarterly and annual documentation for NYS DOH.-Actively participate in weekly staff check-in calls with Program Coordinator and inmonthly staff meetings.-Other duties as assigned by the Program Directors and Program Coordinator.Minimum Qualifications: An Associate's degree in human services or related field is preferredwith a minimum of 2 years of relevant experience. The education and experience requirementmay be satisfied by an equivalent combination of education and experience; for example, a highschool diploma (required) with a minimum of four years of experience coordinating wellness orrecreational activities for adultsSpecial Qualifications: There must be demonstrated ability to interact with people who areexperiencing physical and/or emotional stress, and the ability to deal tactfully with others.Demonstrated interpersonal, critical thinking and communication skills, and experiencenetworking with community agencies for outreach and promotion of services. The positionrequires the ability to use basic office equipment and software for the purposes of data trackingand reporting.
