　　We are currently hiring a Care Coordinator - Saratoga, to work in our Care Coordination Services Agency. We are looking for someone who enjoys working in a fast paced and team environment. This position is for someone who is a problem solver and enjoys helping those in need. As the Care Coordinator, you would be responsible for:

　　Assess and develop care plans to assist clients in addressing gaps in care and social determinants of health

　　Coordinate the care of those with two or more chronic illnesses

　　Refer and link clients to needed services and resources

　　Work with high-risk populations including clients with, a mental health diagnosis, homelessness HIV/AIDS, Sickle Cell Anemia and/or Substance Use Disorder

　　Ability to work in a fast-paced environment

　　Complete proper documentation

　　Care Coordinator - Saratoga Minimum Salary: $19.23 hourly

　　Care Coordinator - Saratoga Maximum Salary: $21.02 hourly

　　Care Coordinator - Saratoga Schedule: Monday-Friday, 8:30am-4:30pm

　　Qualifications:

　　Bachelor’s Degree (preferred but not required) in Health, Education, or Human Services

　　1-3 years of case management experience

　　Experience working with diverse populations
