Position Summary: This position is responsible forhelping our individuals understand, navigate, and grow through the CCSprogram. This Care Coordinator meets and works with the individuals,service providers, and other team supports that assist in theindividual\'s progression through the CCS program. The Care Coordinatorreceives consultation from mental health and substance abuseprofessionals while working on the individuals\' individualized recoveryplan. Essential Functions: Establish individual\'s eligibility by usinga Mental Health/AODA Functional Screen. Assist the individual inidentifying services from the CCS service array appropriate to addressrecovery needs. Broker services as needed with other providers in theCCS network. Work with the individual to identify and build a recoverysupport team of professionals as well as natural supports. Work with theindividual to make an individualized recovery treatment plan. Facilitaterecovery meetings with the individual and the team. Work with theindividual to prioritize and obtain the goals set forth in theindividual recovery plan.
