Job Description:
Case Managers serve as the primary point of contact at BHC for a caseload of adults diagnosed with serious mental illness.Duties involve assisting clients develop their personal recovery plan, completing clinical assessments to determine needs and measure progress, making referrals to other agencies, providing skills training to help clients achieve their personal goals by learning to better manage symptoms of their illness.Case Managers provide crisis intervention.Case Managers provide services in the office and in the field, helping ensure clients physical needs are being met and helping facilitate access to things that are important to success in their behavioral health treatment.To effectively perform duties, Case Managers need strong organizational skills, computer skills, and consistent ability to demonstrate compassion and collaboration with clients, families, staff within the agency and community partners.Qualifications:
Bachelor's degree from an accredited college or university in a social, behavioral, human service or health-related field including, but not limited to, psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice.
Prefer one year of full-time work experience in the human services field, preferably including case management experience.Requires work or volunteer experience or expertise in one or more of the following areas: employment acquisition, housing, budgeting, community resource acquisition, etc. and the ability to communicate that knowledge to others.Must possess a current Texas driver's license and qualify for Center coverage.Must have an insured personal vehicle to be used for Center duties/responsibilities.This is a full time position. Case Managersearn a minimum of$17.45 - $19.88per hour(DOE) with step increases at one and two years. Full benefits available.
