GENERAL DESCRIPTION:
This position is responsible for the ongoing monitoring and implementation of the level of care assessment for participants who have an acquired brain injury on the Supports and Comprehensive waivers and all participants' Community Choices Waivers which are overseen by the Home and Community Based Waiver Programs. The level of care coordinator works to verify waiver participants receive their level of care assessment as needed, completes reconsiderations if warranted, and tracks all level of care activities regarding participants LT101 within the Medicaid waiver system. The individual in this position will also serve as one of the agency’s subject matter experts regarding the Community Choices Waiver and assessed level of care based on the Wyoming Medicaid Rule and the HCBS waiver agreement with the Centers for Medicare and Medicaid Services.

Assure that each participant that requires an LT101 level of care assessment is completed upon eligibility and annually thereafter while the participant is receiving waiver services through the Community Choices Waiver and participants who have an Acquired Brain Injury and receive services on the Support and Comprehensive waiver.
Provide quality customer services and technical assistance regarding level of care assessments to current County Public Health Nurses, waiver case managers, and participants, colleagues, providers, stakeholders, and the public.
Communicate with necessary parties regarding changes in rules, procedures, expectations, and goals while ensuring compliance with federal and state regulations.
Provide ongoing training with Public Health Nurses regarding the administration of LT101 level-of-care assessments.
Work with the designated state Public Health Nurse to coordinate training and needed technical assistance regarding processes and ongoing requirements.
Coordinates with Unit Manager ongoing LT101 activities and other duties as assigned.
Qualifications
PREFERENCES:
Associate's degree in Social work, Human Services, Psychology, or other related field or equivalent relevant experience.
KNOWLEDGE:
Excellent Verbal and written communication skills.
Conflict resolution skills
Ability to multi-task and manage changing workloads and expectations
Knowledge of resources in the State and communities and non-waiver services that may assist participants in obtaining needed services.
Ability to analyze rules and procedures to make independent judgments
Knowledge of current Medicaid rules and procedures
Knowledge of current rules of the Wyoming Office of Administrative Hearings
Knowledge of requirements within current Community Choices, Comprehensive, and Supports Waiver documents
Knowledge of current practices in the field of Developmental Disabilities
Knowledge of community resources for services
Familiarity with other agencies who serve individuals who are aged, blind, or disabled.
Knowledge of various computer programs
Ability to educate and provide technical assistance to stakeholders regarding HCBS Waiver requirements
Ability to educate and provide technical assistance to stakeholders regarding Medicaid Rules and Regulations related to the Community Choices, Comprehensive, and Supports Waiver.
Ability to troubleshoot concerns within these programs
Ability to analyze data and work with other Division staff to remediate identified problems.
Ability to prioritize work to meet deadlines.
Know and comply with HIPPA policy and procedures.
Enter, correct, and monitor data in electronic systems, including input, verification, and quality checking.
Knowledge of all State databases, knowledge of State and Federal law, office coping skills, and teamwork.
MINIMUM QUALIFICATIONS:
Education:
Bachelor's Degree (typically in Human Services)
Experience:
0-1 year of progressive work experience (typically in Benefits and Eligibility)
OR
Education & Experience Substitution:
3-4 years of progressive work experience (typically in Benefits and Eligibility)
