We are seeking an experienced Aging Care Manager 2 to join our team. As an Aging Care Manager 2, you will play a vital role in providing high-quality care and support to elderly individuals. DESCRIPTION OF WORK
As an employee in this position, you are expected to provide information & referral service at the Agency's office site via telephone, computer, fax and mail and/or office visits. Obtain pertinent information through direct contact with consumers, members of the consumers' families, professionals and other persons necessary in order to evaluate the information and to determine consumer needs and the appropriate course of action. Provide informational & educational programs to senior groups, civic clubs or any interested organizations in Westmoreland County. Provide in-service training for home health agencies, subcontractors and other social service agencies upon request. Set up and staffs agency exhibits at health fairs, shopping malls and other public service events. Provide information on services and make referrals by completing intake forms & screening tools.
Work Schedule and Additional Information:Full-time employment.Work hours are 8:30 AM to 4:00 PM, Monday - Friday, with 30-minute lunch.May be required to be on standby rotation for off-hours coverage.You will receive further communication regarding this position via email. QUALIFICATIONSMinimum Experience and Training Requirements:Six months as an Aging Care Manager 1 or a County Caseworker 1; orSuccessful completion of the County Social Casework Intern program; orSix months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; orAn equivalent combination of experience and training.
