DESCRIPTION OF WORKIn this position, you will be expected to complete a comprehensive functional abilities assessment in order to determine the appropriate level of care for individuals wanting to apply for nursing facility care, personal care and domiciliary care placement, as well as home and community-based services. This requires the ability to utilize proven interview techniques in order to gain accurate information from individuals and their representatives. They must possess the ability to manage a large number of assignments each month, in regard to scheduling and completing assessments within the mandated time frame, and the necessary follow-up.
Work Schedule and Additional Information:Full-time employment.Work hours are 8:30 AM to 4:00 PM, Monday - Friday, with 30-minute lunch.May be required to be on a standby rotation for off-hours coverage.You will receive further communication regarding this position via email. QUALIFICATIONSMinimum Experience and Training Requirements:Six months as an Aging Care Manager 1 or a County Caseworker 1; orSuccessful completion of the County Social Casework Intern program; orSix months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; orAn equivalent combination of experience and training.
