The Advanced Patient Care Pharmacist demonstrates competency across all areas of pediatric clinical pharmacy practice, and provides age-appropriate care to patients from the age of birth through 21 years, and select adult patient populations. A Place to Be Proud.ResponsibilitiesServes as a member of the patient care team as the pharmacotherapy expert, such as: rounds with the medical team to prospectively recommend and evaluate individualized drug therapy, makes recommendations to achieve best outcomes in an evidence-based and patient-specific manner while considering pharmacoeconomic principles, and prospectively identifies potential adverse drug reactions, drug-drug, drug-disease and drug-food interactions, and incompatibilities affecting patient outcomes.Coordinates and maintains the antimicrobial stewardship program, including:Overseeing daily stewardship operationsCreate agile and innovative programs to imporove antimibrobial utilization throughout the organizationMaintain antimicrobial formulary and work with infectious diseases service line to evaluate appropriate use of restricted antimirobialsDevelop pathaways for appropriate antimicrobial use in collaboration with the infectious disease teamMonitor and trend antimcrobial usage throughout the Nationwide Children's Hospital organization, including inpatient, ambulatory and accountable care areasDocuments direct patient care, clinical, operational, and financial outcomes and activities appropriately in the medical record.Provides appropriate handoff for pharmacists and other healthca
