　　Purpose: The person in this position will provide support to the Grace Hospital Liaisons.       

　　Job Summary:

　　Track referrals working with the team and Intake to ensure  that all information is given to Intake so referral can be processed

　　Meet with families to secure consents, explain the hospice benefit and ensure  expectations are set

　　Work with hospital Case Management and SW’s along with the on-site  Grace team

　　Order DME and equipment at the direction of the Grace RN

　　Other duties as assigned within the persons scope of practice by the Grace Hospital Liaisons

　　Ensure that Grace Healthcare Services’ Best Practices are incorporated into all job activities, and responsibilities.

　　Qualifications:

　　College degree in the healthcare field is a plus

　　Must have healthcare experience

　　Excellent customer service and communication skills

　　Ability to multitask

　　Organizational skills
