This Administrative Program Specialist A position will serve as a Care Coordinator for the agency, and there are multiple vacancies to be filled.
These positions may work in a centrally located area, or across our outpatient behavioral health facilities, which may include clinics in the following locations:
Hammond (Tangipahoa Parish)
Slidell (St. Tammany Parish)
Mandeville (St. Tammany Parish)
Denham Springs (Livingston Parish)
Bogalusa (Washington Parish)

Salary may be negotiable depending on the chosen applicant's level of experience. Below is a list of the pay range for the position :
Administrative Program Specialist A: $17.18/hour minimum - $33.71/hour maximum

Qualifications
MINIMUM QUALIFICATIONS:
A baccalaureate degree.
SUBSTITUTIONS:
Six years of full-time work experience in any field may be substituted for the required baccalaureate degree.
Candidates without a baccalaureate degree may combine work experience and college credit to substitute for the baccalaureate degree as follows:
A maximum of 120 semester hours may be combined with experience to substitute for the baccalaureate degree.
30 to 59 semester hours credit will substitute for one year of experience towards the baccalaureate degree.
60 to 89 semester hours credit will substitute for two years of experience towards the baccalaureate degree.
90 to 119 semester hours credit will substitute for three years of experience towards the baccalaureate degree.
120 or more semester hours credit will substitute for four years of experience towards the baccalaureate degree.
College credit earned without obtaining a baccalaureate degree may be substituted for a maximum of four years full-time work experience towards the baccalaureate degree. Candidates with 120 or more semester hours of credit, but without a degree, must also have at least two years of full-time work experience to substitute for the baccalaureate degree.
NOTE: Any college hours or degree must be from an accredited college or university.

Differs from Administrative Specialist--B by the absence of a moderate degree of complexity, impact, or independence in assigned duties and program areas.
Examples of Work
This position will serve as a Care Coordinator team member working directly with Behavioral Health and Primary care Management teams, which provides care coordination services to the entire service catchment area. The primary function of this position is to independently assist persons served with access to care, help persons served navigate the complex health care system, and to ensure the quality of their care by monitoring clinical outcome measures and developing recommendations related to improvement measures. The Care Coordinator works alongside a multidisciplinary health care team (which may include the Medical Director, Psychiatrists, Medical Psychologists, Psychiatric Mental Health Nurse Practitioners, Primary Care Nurse Practitioners, RNs, LPNs, pharmacists, and clinicians) in order to ensure coordination of care and appropriate communication of care among multiple disciplines. This position will also identify and manage clients’ chronic conditions and needs, including behavioral health, preventive health, and clinical measures to be implemented. The coordination of care will include face-to-face, virtual, and telephone contact with clients, family, caregivers, physical care team, specialty care team, and other providers and involves secure sharing of information to promote safe and effective care.
