Job Description:
Astor’s Rockland Adolescent Partial Hospitalization Program is seeking a proactive, highly motivated, and organized Administrative Case Manager to join a collaborative, fast-paced, high-performing team that provides clinical and psychiatric services to youth working to remain out of inpatient hospitalization. The Administrative Case Manager will be responsible for receiving referrals as well as triaging to coordinate intakes and discharge planning. The Administrative Case Manager will obtain consents, schedule appointments, and send annual notices to families (mailing & electronic correspondence). They will complete program satisfaction surveys for clients and families and be responsible for 30 day follow up communications. They will submit financial requests, and track/submit QA reporting data (including but not limited to fire drill reports, restraint data, training information, emergency preparedness, information, etc.) into the SharePoint. The Administrative Case Manager will maintain data in Microsoft Excel such as admissions, discharges, and other reports. They will be required to assist supervisors, clinicians, and other team members to complete duties as directed to support the efficiency of program functioning.
