A Brief OverviewThe Center for Health Equity (CHE) Health Navigator will utilize exceptional customer service skills to engage, support, and educate families in connecting to healthcare and social needs resources. The CHE Health Navigator will act as a healthcare and community liaison to support patients and families in navigating the healthcare system and community resources to meet their needs. This includes educating caregivers regarding making appointments both in-person and via telehealth. They will educate patients and families in downloading, connecting and navigating virtual visits for appointments and resources in order to successfully navigate and connect families to community resources. They will work in both clinical and community-based settings, making home visits to connect families to needed resources and communicating family needs with the larger care team. CHE Navigator will support patients and families to ensure ability to complete telehealth encounters as needed.
What you will doAssist patients and families in identifying education, information, referral, and advocacy related to the management of their social needs.Work to reduce cultural and socio-economic barriers between patients and institutions.Refer families to community resources, social services, insurance providers, and other relevant providers for patients and families recommended or identified by the care team.Continuously expands knowledge and understanding of community resources and servicesEngages families in obtaining self-management skills for disease managementUtilizes high-level engagement techniques to establish rapport with patient familiesCommunicates identified family needs to the larger care team, including the social workerUses Language of Caring in all intersections with patients/families, particularly those who are struggling with the technologyProvide clear and concise documentation of patient/family interactions and home visitsEducation QualificationsHigh School Diploma / GED Required Experience QualificationsAt least three (3) years of community-based health work, working in the pediatric community, care coordination or case management Required Skills and AbilitiesAbility to travel by car or independence on public transit, taxi, or ride-sharing programKnowledge of local community agencies and organizations.Strong interpersonal skills, communications skills, problem-solving skills, teaching and coaching skills, and community networking skills.Ability to work effectively with a wide range of constituencies in a diverse community.Personal experience with medical complexity preferred but not requiredOutstanding customer service and communication skillsAbility to work independently and in collaboration with other teamsAbility to navigate Smartphones and mobile devices; IOS and AndroidExcellent computer skills; mail, Word, EPICUnderstands and values the importance of equitable access to healthcareTo carry out its mission, CHOP is committed to supporting the health of our patients, families, workforce, and global community.
