JOB REQUIREMENTS: Job Description: Title: Service Coordinator I NationalChurch Residences- All Divisions Status: Non-exempt Reports to: PropertyManager/Portfolio Manager for Enriched Housing Services/ Program ManagerRevision date: August 2022 Supervises: n/a PURPOSE According toprescribed policies and procedures of the organization including allapplicable federal, state, accreditations, and regulations and under thegeneral supervision of the Property Manager/Staffing Manager/ProgramManager, the Service Coordinator assumes responsibility for coordinatingprograms and services to help participants maintain a good quality oflife and age in place. The Service Coordinator prioritizes aperson-centric model of service delivery. ESSENTIAL FUNCTIONS AND BASICDUTIES 1. Assumes responsibility for coordinating programs andactivities for eligible individuals on a group basis. a. Serves as aliaison to community agencies, network with community service providers,and seek out new services available to eligible individuals. Identifylow cost service providers and/or negotiate discounts. b. Monitors theongoing provision of services from community agencies and appropriatelydocuments participant\'s use in the designated documentation managementsystem. Manages the provision of supportive services where appropriate.c. Develops and maintains a Resource Directory that includes a listingof state and/or local service providers. Examples include services tofamilies, children, elderly individuals, persons with disabilities andemergency assistance. d. Organizes educational events that includesubjects relating to health care, agency support, life skills andreferral sources. Promotes participation and links eligible participantsto events. e. Identifies partners to provide health services andscreenings to participants at their home. f. Assists participants inbuilding informal support networks with other participants, peers,family, and friends. g. Engage volunteers within the community and/orproperty when appropriate based on National Church Residences VolunteerPolicies and Procedures. 2.Assumes responsibility coordinating servicesfor participants on an individual basis through a person-centered,non-clinical assessment, intervention, and monitoring process. a.Outreaches to all individuals eligible for the program to provideeducation about the program and offer enrollment.
