Job Description:
Under general supervision, Level IV Clinical Specialist Pharmacists have the expertise and advanced knowledge to be viewed as a system-level content expert in a clinical domain. Their advanced knowledge and practice experience facilitate the delivery of safe, effective, and appropriate medication therapy to their assigned patient populations. Level IV Clinical Specialist Pharmacists actively participate in interdisciplinary committees, staff education, professional development, performance improvement initiatives, and oversight of pharmacy residents when applicable. Level IV Clinical Specialist Pharmacists have a dedicated clinical role with opportunities to manage and monitor medication therapy as part of a direct patient care team, usually in a specific patient population. In addition, they will play a major role in the education of pharmacy students, pharmacy residents, medical residents, providers, nurses, and ancillary staff.
Duties and Responsibilities
Provides medication therapy management services in collaboration with other members of the health care team including consultative services pursuant to the department’s applicable services which may include:
Pharmacokinetic evaluation, monitoring, and dosing (initiation and daily follow-up)
Total parenteral nutrition (TPN) consults
Anticoagulation initiation and follow-up pursuant to a consult
Renal dose adjustments
Intravenous to oral conversions
Additional clinical opportunities may be available based upon departmental needs, clinical experience, and performance, and may include unit-based pharmacist, antimicrobial stewardship, patient care rounds, or pain management team
Direct patient care as part of interdisciplinary team, if applicable, through development of individualized treatment and monitoring plans for patients
Assists with medication reconciliation during all changes in level of care, including but not limited to, transfers and postoperative care
Provides educational services to patients and healthcare providers regarding medication therapy and/or medication safety concerns.
Participates in the training and education of pharmacy technicians, pharmacy students and/or residents, and medical residents. Fulfills and maintains American Society of Health-System Pharmacists (ASHP) preceptor criteria for accredited residency programs, including participation in ongoing departmental preceptor development.
Provides medication event prevention, monitoring, and reporting, with an increased emphasis on patient safety.
Utilizes technology to enhance patient safety by ensuring the proper distribution, monitoring, and use of medications; participates in the assessment and maintenance of technology.
Participates in quality, safety, and performance initiatives related to medication use at the organizational and departmental levels. Takes a lead role in collaboration on departmental process research activities, including mentoring of other staff through research initiatives.
Adheres to WellSpan Health’s medication-use policies and standard work. Develops and modifies polices and standard work related to the use of medications within the health system. Participates in the health system’s formulary process, ensuring an appropriate formulary system is in place. Oversees non-formulary medication review.
Actively participates on department, facility, or health-system committees, including the Pharmacy and Therapeutics Committee and Drug Shortage Committee and proposes drug shortage management plans.
Leads or collaborates in clinical system/process research activities. Contributes to practice literature.
Qualifications
Minimum Experience:
Completion of PGY-1 residency with 3 years of hospital experience AND/OR PGY-2 residency with 1 year of hospital experience; OR 7 years hospital experience; OR Internal advancement from Level III Pharmacist position
Minimum Education:
Pharmacy Bachelor's Degree
Required Certification:
Valid Pennsylvania Pharmacist License or eligible for reciprocity
Preferred Certification:
Board of Pharmaceutical Specialties (BPS) certification preferred
