About this position Help individuals to identify and overcome barriers preventing successful compliance with their individual care plans by monitoring, evaluating, and educating customers in their home environment. The Community Paramedic will serve as customer advocates and resource coordinators in order to facilitate identification of customer needs and access to available health and community services. Bring appropriate care and supportive services directly to the customer/client out in the community empowering them to reach their health care goals and optimal wellness. What we'll expect from you * Conducts post-hospital release follow-up care including, but not limited to, medication reconciliation/ monitoring, dressing changes, checking vital signs. * Conducts customer education including but not limited to, diabetes prevention/treatment, hypertension, Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), and nutritional assessment and education. * Provide health/injury preventative services, falls/home safety assessment, geriatric frailty visits, vaccinations, serve as community resource contacts. * Collect specimens for laboratory analysis as ordered, perform simple point of care testing if indicated. * Determine social supports necessary to help customer maintain their health. * Completes appropriate documentation and templates for the Community Paramedic Program, clearly communicates customer progress, concerns, outcomes with accurate, concise reports. * Coordinates customer care with all members of care coordination team, coordinates appointments and follow-up with physicians, therapists, and hospitals. * Accepts other duties as assigned to promote the accomplishment of organizational goals. * The team member is accountable to demonstrate proficiency for the skills outlined in the appropriate position skills list. * The team member is accountable to maintain skill proficiency, including improvement where deemed necessary, and upgrading any additional or new skills on the appropriate position skills list. * Recognizes and complies with legal, regulatory, accrediting and procedural requirements related to area of responsibility. * Understands and follows principles and standards as outlined in North Memorial's Corporate Code of Conduct. * Protects customer and team member privacy and only accesses customer and/or team member related information as needed to perform job duties. * Reports violations or areas of concern to supervisor or Corporate Compliance Officer via established methods of communication. * Understands and follows principles and standards as outlined in North Memorial's Corporate Code of Conduct. * Protects customer and team member privacy and only accesses customer and/or team member related information as needed to perform job duties. * Reports violations or areas of concern to supervisor or Corporate Compliance Officer via established methods of communication. * Recognizes and complies with legal, regulatory, accrediting and procedural requirements related to area of responsibility. * Understands and follows principles and standards as outlined in North Memorial's Corporate Code of Conduct. * Protects customer and team member privacy and only accesses customer and/or team member related information as needed to perform job duties. * Reports violations or areas of concern to supervisor or Corporate Compliance Officer via established methods of communication. Here are ou requirements Education * High School degree or equivalent (GED) required * Successful completion of Community Paramedic program * AAS Paramedic degree preferred Experience, Knowledge, Skills, Abilities * 2 years ALS experience as a Paramedic * Be comfortable and motivated to work as a care team member and autonomously with a high level of accountability. * Possess superior interpersonal communication skills. * Demonstrated ability to consistently complete through, concise, detailed documentation. * Demonstrated ability to organize, problem solve, and make decisions during emergency situations and work independently. * Possess or quickly acquire knowledge of the street and highway system within the service area. Licensure/Certification(s) Mandatory * Minnesota EMT-P Certificatio- State Requirement * Certified Community Paramedi- State Requirement * ACLS/BLS Certificatio- State Requirement * Valid Minnesota Driver's license and acceptable driving record - NMHC Standard Licensure/Certification(s) Mandatory where Applicable Licensure/Certification(s) Preferred Hours per two week pay period: 20 Shift: Day/Evening 10 hours Weekend Requirement: Weekends by Department Rotation Work Group: Non Contract Call Requirement: None Remote or On-site: On-site FLSA Status: Non-Exempt Benefits: No Equal Opportunity Employer/Protected Veterans/Individuals with Disabilities.
