  This position will receive a short-staffing temporary pay differential of (3.17%) from July 1st, 2023 - June 30th, 2024.

  COLORADO STATE VETERANS COMMUNITY LIVING CENTER – HOMELAKE:

  To ensure a safe, sanitary, well-maintained, comfortable living and working environment for residents of a 60-bed skilled nursing home and 50-unit domiciliary, staff, and visitors. The facility has embraced the Culture Change and Eden concepts. We strive to provide exemplary service to the residents by fulfilling daily and special requests that the residents may have. Exemplary care is given in cleaning and sanitizing and in keeping the resident rooms and their atmosphere clean, organized, homelike, and safe.   Description of Job

  This position is a full-time, permanent, in-person position. Shift work is required: Must be able to work a flexible schedule including weekends, holidays, evenings, nights, and days as needed.

  Please note: These working arrangements are subject to change.

  This work unit exists to provide continuous 24-hour care to nursing home residents requiring skilled nursing services and providing services to the 50-unit domiciliary as needed. The goal is to maintain residents at their highest level of functioning and provide quality nursing care. This position provides basic nursing care to residents of varying mental, physical, and health statuses. The position is responsible for meeting the needs of the residents (ie., activities of daily living, perform ambulation, range of motion, vital sign assessments, etc.).

  Primary Job Duties Include:

  Performs basic nursing care; bathing, feeding, dressing, transferring, ambulating, positioning, lifting, range of motion, toileting, skin care, etc.

  Safety of residents and others by use of correct lifting techniques, monitoring the environment and Resident whereabouts/activities, use of safety devices such as lifts and enabler bars, etc.

  Performs basic resident assessments such as weights, bowel and bladder incontinence, intake and outputs, and vital signs, and records the same.

  Records hall data sheet each shift.

  Implements proper infection control practices via the use of standard precautions.

  Communicates with others to ensure proper resident care and work completion.

  Keeps RNs and LPNs informed of findings and changes in resident condition.

  Maintains therapeutic milieu through appropriate interpersonal skill use, monitoring of environments, and assisting residents to maintain rooms and possessions.

  Maintains resident care and equipment such as cleaning oxygen concentrators, filling portable oxygen tanks, maintaining/disinfecting tubs and showers, changing lift batteries, etc.

  Transports residents for medical appointments, etc. in state vehicles when the need arises.

  Attends in-house and off-site educational training, cornerstone online training, and staff meetings to maintain current techniques, policies adequate inter-shift communication.

  Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights

  MINIMUM QUALIFICATIONS:

  Education and Experience:

  A high school diploma or GED

  AND

  At least one (1) year of professional experience as a CNA or performing similar duties/assignments of a CNA.

  Employment history is calculated on a full-time basis (40/hours per week). Part-time employment is calculated on a prorated basis to determine qualifications. Be sure your application specifically addresses your qualifications, experience, work products, and accomplishments as they relate to the position and minimum requirements.

  Substitutions:

  Current, valid nurse aide license (CNA) from the Colorado Department of Regulatory Agencies will substitute for the high school diploma or GED.

  Current, valid licensure as a Practical Nurse (LPN) from the Colorado Department of Regulatory Agencies or any state participating in the Nurse Licensure Compact will substitute for the experience requirement.

  Required Competencies

  Skill in initiating, performing, and completing assigned duties in providing care to residents.

  Possess good verbal, listening, and written communication skills with all health care providers, residents, families, and multidisciplinary team members.

  Demonstrated ability to display professional conduct at all times when dealing with fellow staff members, residents, families, and the general public.

  Demonstrated ability to maintain confidentiality at all times.

  Long-term care/nursing home experience is highly desirable.

  Effective problem-solving skills.

  Preferred Competencies

  Knowledge and ability to appropriately carry out assigned resident care based on the residents' conditions; to use judgment and to accurately recognize report, and record relevant resident information.

  Knowledge and ability to explain assigned resident care best practices to peers with less experience.

  Knowledge and ability to recognize urgent or emergent resident care situations, seek guidance and direction of the RN and/or Supervisor/DON, and initiate appropriate emergency interventions as directed.

  Knowledge of quality standards, procedures, and protocols.

  Knowledge of specialized client care applicable to the work situation.

  Ability to effectively work with a resident population of varying physical, mental, and emotional abilities and staff from multiple work units in the facility to achieve quality care planning and delivery.

  Conditions of Employment:

  Full Background - CBI name check and fingerprint check, ICON Colorado court database, Medicare fraud database, Reference Checks (Skill Survey), Professional License verification (licensure requirements), Drug Screen, CAPS (direct contact with adults – Mental Health Institutes, Regional Centers, Veterans Community Living Centers)

  Shift Work: Must be able to work a flexible schedule including weekends, holidays, evenings, nights, and days as needed.

  This position is eligible per the Division of Personnel Administration (DPA) pay plan for on-call status.

  This position requires the ability to perform various physical movements, including but not limited to lifting and transporting residents.

  Must possess a valid US Driver's License.

  Other – Current & active Certified Nurse Aide license renewal required and be CPR certified

  Pre-employment Physical: Must pass TB test which will be given before employment at employer's expense.

  Travel – off-site training, transportation

  This position is considered essential services and therefore this employee must report to work without delay or interruption to provide essential or emergency services to ensure the health, safety, and welfare of clients.

  Former State employees who were disciplinarily terminated or resigned in lieu of termination must disclose the information on the application and explain as to why the prior termination or resignation should not disqualify the applicant from the current position. Absent extraordinary circumstances, prior disciplinary termination, or resignation in lieu of termination will disqualify the applicant from future State employment with CDHS.
