The SupportSpecialist serves youth at various stages of their recovery from mentalhealth, substance use, and co-occurring needs by embracing the youth\'sstrengths to meet their identified needs. The Support Specialist rolevaries on each team and services are provided in the community to meetthe needs identified in the Plan of Care. Essential Functions: Act as amember of the CCS team providing services to youth and young with mentalhealth, substance use, and co-occurring needs. Build off the person\'sstrengths to meet their individualized recovery needs. All services willbe person centered and directed by the individual being served. Provideindividual skill development and enhancement services that includetraining in communication, interpersonal skills, problem solving,decision making, self-regulation, conflict resolution, and otherspecific needs identified in the Plan of Care. Services also includetraining in daily living skills related to personal care, daily tasks,transportation, school, and accessing and connecting to communityresources and services (including healthcare services) and otherspecific daily living skills identified in the Plan of Care. Utilizevarious methods to serve people including but not limited to; rolemodeling, monitoring, mentoring, supervision, assistance, and cuing.Provide services in the community including homes and other communitylocations. Participate as a team member in team meetings with theindividual to create a Plan of Care, identify and prioritize goals, andmonitor progress toward meeting identified needs. Appropriately documentall contact with the people being served and team members to adhere toprogram requirements. Responsible for accurate documentation andmaintaining a case file as required by agency policy, governmentregulations, and applicable local, state, and federal codes.
