About Wound/Skin Care Coordinator

  Description Summary: The facility Wound/Skin Care Coordinator will provide clinical oversight and coordination for wound care services by doing weekly wound rounds on all units, documenting on weekly wound flow sheet, communicating with MD weekly on wound status (unchanged, improved, deteriorated and family as warranted, updating care plan weekly to reflect current wound/skin status and interventions (non-pharmacological and pharmacological) and updating nursing accountability record weekly as needed. The facility Wound/Skin Care Coordinator will provide consultation, educational support and staff development activities that provide the bedside nurse with the knowledge, skills, and confidence to manage wounds will be critical to the success of this position. In addition, the wound care coordinator may provide consultative and educational services to residents and their family members to assist them in the management of their individual wound care needs. Ultimate responsibility and accountability for day to day patient care will continue to rest with the individual nurse responsible for the resident and the Nurse Manager of each unit. The Wound Care Nurse will also serve as a resource to the facility regarding: National Patient Safety Goals related to pressure ulcers and Cost effective, appropriate utilization of wound care dressings, supplies, and pressure reduction specialty beds.

  Requirements: The facility Wound/Skin Care Coordinator will be able to demonstrate the following essential competencies, duties and responsibilities as a result of education, experience and NHC orientation, and ongoing corporate learning.

  CLINICAL: 1. Adhere to established medical/ nursing practice standards and facility policies and procedures2. Provide wound care consultations and educational support to residents and their family regarding wound care management.3. Provide clinical support and coordination for wound care services on a weekly and as needed consultative basis.4. Facilitate consultations by specially trained wound care physicians as necessary.5. Contribute to nursing orientation with a focus on skin assessment, documentation and prevention of pressure ulcers.6. Function as a resource to staff for wound product selection and use.7. Assist in the development of key clinicians to facilitate the skin and wound care program.Contribute to discharge planning meetings on units managing patients with wound.MANAGEMENT/EDUCATION:1. Develop and provide staff training to develop wound care skills for the bedside nurses.2. Contribute to the needs assessment and competency development related to wound care for clinical staff providing direct patient care.3. Participate in the facility multidisciplinary wound care rounds.4. Contribute to nursing orientation with a focus on skin assessment, documentation and prevention of pressure ulcers.5. Monitor the appropriate use of specialty beds using criteria approved by the hospital.IMPROVE PERFORMANCE:1. Coordinate incidence and prevalence study and participate in risk management/performance improvement activities aimed at improving the incidence and severity of hospital acquired pressure ulcers.2. Participate in the review and revision process for hospital policies and procedures relating to wound care.3. Participate in the review and selection of all specialty supplies, dressings, and materials for wound care to facilitate cost effective utilization and compliance with vendor contracts.4. Participate in the review and revision process for hospital policies and procedures relating to wound care.COMMUNICATE: 1. Demonstrate excellent skills in the area of interpersonal relations. This relationship includes the ability to coach other resident care personnel in the delivery of patient care. The approach to all customers must be sensitive and courteous, maintaining a professional, yet personable relationship. This position has a high impact on the quality of resident care.2. Collaborate with the clinical nurse manager and designee at the Wound Healing Center (when applicable) to assure continuity of care across inpatient and outpatient service lines, to include referrals to the outpatient Wound Healing Center upon discharge.3. Provide monthly reports to the Admiistrator, DNS and Corporate Director of Nursing and that summarize consultative, educational, and staff support services provided.4. Follow proper channels of communication anticipating and consistently handling daily routine problems while recognizing complex issues that need referral.5. Support and encourage harmonious working relationships and team building skills.6. Consistently demonstrate openness to receiving and giving constructive feedback.7. Identify problems/concerns and discuss them professionally.GENERAL: 1. Adjust scheduled time off to meet department needs.2. Ensure that time and attendance policies are followed.3. Adhere to facility policies and procedures.1. Qualifications:At least one year of experience in wound care and acute care experience. Case management and discharge planning experience are preferred. Certification, Registration, or Licensure: Current R.N. license; current CPR certification for the Healthcare Provider (BLS); Wound Certification preferred (AAWM or WOCN certifying bodies preferred). Demonstrate a high degree of autonomy, organization and flexibility in scheduling to assure patient, family, and staff needs are met.2. Supervisory:The incumbent in the position will not or will supervise other employees.3. Education/Experience:Graduate of an accredited School of Nursing4. Language Ability:Ability to read, analyze, and interpret general business periodicals, professionaljournals, technical procedures, or governmental regulations. Ability to write reports,business correspondence, and procedure manuals. Ability to effectively presentinformation and respond to questions from groups of managers, clients, customers, and the general public.5. Math Ability:Ability to work with mathematical concepts such as probability and statisticalinference, and fundamentals of plane and solid geometry and trigonometry. Ability toapply concepts such as fractions, percentages, ratios, and proportions to practicalsituations.6. Reasoning Ability:Ability to apply principles of logical or scientific thinking to a wide range ofintellectual and practical problems. Ability to deal with nonverbal symbolism(formulas, scientific equations, graphs, musical notes, etc.,) in its most difficult phases. Ability to deal with a variety of abstract and concrete variables.7. Computer Skills:Must be able to use Microsoft Outlook, Word, PowerPoint, Excel, as well as web-based database programs and the Internet.8. Work Environment:The incumbent in this position is not required to work under any extreme conditions. 9. Physical Demands:The incumbent in this position typically has to stand approximately 40% of the time,walk 50% of the time and sit 10% of the time. In addition, the person is using touch or feel 80% of the time and reaches with his arms or hands approximately 80% of the time.
