Position: Transition Facilitator - 1 Full Time or 2 Part Time Available

　　Minimum Education and Skills

　　Bachelor’s degree in human services related field.

　　Two years of experience in working with children, adolescents, young adults, their families, and/or adults as a case manager or other similar role.

　　OR

　　A Master’s degree in a human services field.

　　At least one year of experience in working with children, adolescents, young adults, their families, and/or adults as a case manager or other similar role.

　　Job Description Title: Transition Facilitator Department: Care Coordination Title of Immediate Supervisor: Transition Program Supervisor POSITION SUMMARY: The Transition Facilitator uses strength based, trauma informed, and family driven evidence based practices to support youth ages 16-17 (up to the age of 23) with successfully building independent living skills. The Transition Facilitator will carry a caseload of approximately 15 youth, and maintain a flexible schedule to respond to the needs of these youth. The Transition Facilitator will work collaboratively with ECDSS and Probation teams with the goals of reducing recidivism in the juvenile justice system. RESPONSIBILITIES/DUTIES: • Maintain a caseload of approximately 15 youth. • Engage young people through relationship development, person-centered planning, and a focus on their futures. • Tailor services and supports to be accessible, coordinated, appealing, non-stigmatizing, trauma-informed, and developmentally-appropriate -- and building on strengths to enable the young people to pursue their goals across relevant Transition Domains. • Acknowledge and develop personal choice and social responsibility with young people. • Ensure a safety net of support by, as relevant, involving a young person’s parents, family members, and other informal and formal key players. • Enhance young persons’ competencies to assist them in achieving greater self-sufficiency and confidence. • Maintain an outcome focus in the TIP Model® at the young person, program, and community levels. • Involve young people, parents, and other community partners in the TIP system at the practice, program, and community levels. • Maintain a flexible work schedule, including evenings and weekends. • Input and update all necessary documentation in Netsmart myEvolv and CONNECTIONS including Assessments, Plans of Care, Safety and Crisis Plans and Progress Notes • Maintain accurate and timely case records, including progress notes and tracking mechanisms, electronically and in each youth’s file • Keep all youth and family information, verbal and written, confidential • Utilize data and outcomes to inform decision making with families • Participate in quality improvement efforts to make changes to individual and team practices • Evaluate efforts with supervisor and team to determine if interventions were successful, or if alternate interventions should be attempted • Collaborate with team members for case coverage and consultation • Participate in regularly scheduled team meetings • Provide crisis response/support for youth and families Agency Standards/Norms: • Participate in regular supervision (per New Directions policy and procedure) • Attend all mandatory in-service trainings as per NDYFS policy and ECDSS requirements • Participate in continuing education as needed to maintain a level of expertise in the field. • Adhere to agency and department policies and procedures • Utilize normative culture • Adhere to the Code of Ethics prescribed by the National Association of Social Workers • Perform all other duties as assigned by immediate supervisor SKILL, QUALIFICATIONS, KNOWLEDGE, ABILITY & PHYSICAL REQUIREMENTS: Minimum Education and/or Professional Qualifications/Skills Degree: A Master’s degree in human services Experience: at least one year of experience in working with children, adolescents, young adults, their families, and/or adults as a case manager or other similar role. OR Degree: A Bachelor’s degree in human behavioral science which includes 30 semester or 45 quarter hours either in development of human behavior, child development, family intervention techniques, diagnostic measures, or therapeutic techniques, such as social work, psychology, sociology, guidance and counseling, and child development. Experience: 2 years of experience in working with children, adolescents, young adults, their families, and/or adults as a case manager or other similar role.
