JOB REQUIREMENTS: As a Physical Therapist (PT) at Westfields Hospital &Clinic, you will perform comprehensive assessment of therapy patient,develop a plan of care, perform treatment as indicated, assesspatient\'s response to treatment, and assist with discharge planning.Duties include: Utilizes exercises, manual therapy, modalities, andeducation to improve the patient\'s overall mobility, pain, andfunctional ability. Provides education related to the treatment program,disease process, and prevention of further injuries to the patient,family/caregiver, and other members of the healthcare team. Evaluatesinpatient and outpatient physical therapy patients. Establishes a planof care and implement treatment, including exercise, manual therapy,modalities, and education Reassesses response to treatment, alteringplan of care as necessary, to optimize a patient\'s mobility, pain, andfunctional abilities Assists in discharge planning process Documents onelectronic health system in a timely manner Communicates with referringpractitioner, nursing unit, and other departments when appropriateCompletes continuing education requirements, and provides staffin-service of material learned Work Schedule: .9 FTE/Full-time, Monday,Tuesday, Thursday and Friday work schedule with weekend rotation.
