About the Department
The Chicago Center of HIV Elimination, embedded within the University ofChicago\'s main campus in Hyde Park, is located in Chicago\'s HIVepicenter. Of the 20 neighborhoods in the city with high annual HIVdiagnosis rates (greater than 40 per 100,000), 13 of them are no morethan eight miles from the University of Chicago. This area includes 4 ofthe 6 communities with the highest annual diagnosis rates in the city(Washington Park, West Englewood, Greater Grand Crossing and SouthShore). Our position within the hardest hit neighborhoods in Chicagoprovides unique opportunities to advance HIV testing and preventioninterventions locally, providing tangible results to those most affectedand to improve the lives of those living with and without HIV infection.CCHE seeks to eliminate new HIV transmission events over the next 30years (from 2011 to 2041) by using network science to target andintegrate prevention as well as create structural and community-specificinterventions.
Job Summary
The Supportive Services Coordinator will hold a caseload that isassigned through internal referrals from our testing programs andresearch projects. Managing this caseload will consist of callingcommunity members and conducting resource counseling, completing allrelated documentation and notes, and researching appropriate andindividualized resources for community members, specifically resourcesfor people of color and those who identify as LGBQ (lesbian, gay,bisexual, queer) and TGNC (transgender and gender non-conforming).
This position is grant funded.
Responsibilities
Manages retention in care for their caseload and follows thefidelity monitoring protocols in data collection and documentation.This role is part of the Supportive Services team and works with avariety of community members who are referred to Supportive Servicesfor additional assistance.Involves working with community members over the phone or in person.Assists with day-to-day supportive services tasks, such as rapidresponse support for those who are seeking same-day treatment andconducting assessments and safety plans for community members indistress.Prepares for or participate in quality assurance audits conducted bystudy sponsors, federal agencies, or specially designated reviewgroups.Participates in study-related documentation, such as protocolworksheets, procedural manuals, adverse event reports, institutionalreview board documents, or progress reports.Performs other related work as needed.Minimum Qualifications
Education:
Minimum requirements include vocational training, apprenticeships or theequivalent experience in related field (not typically required to have afour-year degree).
