JOB REQUIREMENTS: Duties Duties include the following: Comprehensivereview of documentation within the health record to assign ICD codes fordiagnosis, complications/major complications, comorbid/major comorbidconditions, surgery and procedures for accurate assignment of diagnosisrelated groups (DRG), and/or assigning CPT/HCPCS codes for inpatientprofessional services. Independently review and abstract clinical datafrom the record for documentation of diagnoses and procedures to ensureit is adequate and appropriate to support the assigned codes. Code allcomplicated and complex medical/specialty diseases processes, patientinjuries, and all medical procedures. Abstract, assign, and sequencecodes into encoder software to obtain correct DRG, support medicalnecessity, resolve encoder edits, and ensure codes accurately reflectservices rendered. Review provider health record documentation to ensurethat it supports diagnostic and procedural codes assigned and isconsistent with required medical coding nomenclature. Ensure inpatientcoding staff queries clinical staff with documentation requirements tosupport the coding process. Enter and correct information that has beenrejected. Ensure audit findings have been corrected and refiled.Establish the primary and secondary diagnosis and procedure codes forencounters following applicable regulations, instructions, andrequirements for allowable reimbursement; link the appropriate diagnosisto the procedure and/or determines level of Evaluation & Managementservice provided. Understand the CPT coding system for Third PartyInsurance cost recovery and interpret instructional notations; bundleencounters when appropriate; identify non-billable encounters. Reviewand code assigned Community Care for inpatient or outpatient encountersusing the paper or electronic documentation obtained from non-VAfacilities such as Community Hospitals, Emergency Rooms, militaryfacilities, etc. Adhere to accepted coding practices, guidelines andconventions when choosing the most appropriate diagnosis, operation,procedure, ancillary, or Evaluation and Management code to ensureethical, accurate, and complete coding. Adhere to the coding guidelinesspecific to the Veterans Equitable Resource Allocation (VERA) programthat categorizes all VA patients into specific classes representingtheir clinical conditions and resource needs.
