　　Generations Family Health Center, Inc. is a regional community health center covering a 37-town rural region in eastern Connecticut with sites located in Willimantic, Danielson, Norwich and Putnam. Our dedicated staff provides a full range of primary care services to all ages including medical, dental and behavioral healthcare, as well as care coordination and community outreach.

　　Location: Danielson, CT

　　Job Responsibilities: As a physician, you will provide a full range of primary care services: from physical exams and therapeutic and surgical procedures, to patient education, and ensuring continuity of care.

　　Salary Scale Starting: $220,375

　　Sign-On Bonus: $10,000 Rural Disparity Payment (Danielson only): $5,000

　　M.D. or D.O. degree from an accredited medical school

　　Family Medicine/Internal Medicine residency trained from an accredited institution

　　Board certification or board eligible

　　Eligibility for Federal and State Controlled Substance Licenses
