Job Summary:
The Palliative Care Advanced Practice Provider will see patients with any chronic disease who may need additional support or advanced decision making. They will focus not only on providing comfort in dying, but also on living with the focus on controlling symptoms, supporting emotional, spiritual, and cultural needs and maximizing functional status. This position will see patients at both our Downtown Campus and Community Campus.
Responsibilities include providing expert level clinical care coordination of care via a full range of services that will improve quality of patient care assessing evaluating patients, communication with interdisciplinary team members and maintaining outstanding relationships with families, referring physicians all members of the health care team.
Minimum Qualifications:
NYS licensed/registered NP or PA required. Master’s Degree as a Nurse Practitioner or Physician Assistant current National Certification consistent to the clinical practice also required. Have a NPI number and be Medicare enrolled with a Medicare Identification Number (or PTAN) (or eligible for Medicare Enrollment within 6 months of hire.
CPR-BLS Certification required at the time of appointment.

Advanced Certified Hospice and Palliative Nurse (ACHPN)
