　　Responsibilities

　　Conduct appropriate evaluations, assess home environment and identify equipment needs relative to speech/language function

　　Identify functional speech defects and establish a plan of care to improve patient’s function

　　Administer speech therapy program utilizing specialized therapeutic technique and/or equipment and recommend communication devices/aids as indicated

　　Maintain accurate and timely documentation

　　Participate in case conference as required

　　Ensure compliance with all state and federal legal and regulatory requirements

　　Additional responsibilities as assigned

　　Qualifications

　　Master’s degree in Speech Language Pathology with current licensure to practice in state of operation

　　1+ year experience in Speech Language Pathology

　　Home health experience preferred

　　Commitment to clinical and documentation excellence
