FTE: 1.000000 Shift: 1 Schedule: Monday through Fridayworking 8 hour shifts on first shift Job Summary: Responsible forcoordinating the discharge plan of care for all patients during theirhospital admission. Coordinates the discharge plan starting on admissionand through discharge follow-up. Provides biopsychosocial assessmentsand provides updates that address the patient\'s evolving discharge careneeds to timely move them to the appropriate next level of care.Establishes a plan of care that provides maximum benefit to patients andtheir families in collaboration with the multidisciplinary team in CareCoordination Rounds (CCRs) seven days a week. EXPERIENCE DESCRIPTION: Aminimum of two years experience in an acute care setting is preferred.Casework in medical social work is preferred. EDUCATION DESCRIPTION:Master\'s Degree in Social Work is required. SPECIAL SKILLS DESCRIPTION:Interpersonal skills necessary to obtain information from patients, toprovide counseling to patients/families, and interact effectively withpatients, families and hospital personnel. Analytical skills necessaryto assess patients needs, develop associated discharge planning anddevelop solutions to problems concerning patients mental and physicalwell being. LICENSURE DESCRIPTION: Current State of Wisconsin licensureas a Social Worker is required.
