　　Job Summary and Qualifications

　　The medical SW Case Manager evaluates psychosocial needs and provides community service coordination for identified patient populations, providing support and crisis intervention where indicated. The SW Case Manager acts as a liaison between the patient and community resources/post-acute care providers. The SW Case Manager creates and facilitates timely implementation of discharge plans and arranges post-acute interventions identified in the plan of care. The SW Case Manager facilitates patient movement across the continuum of care and identifies, addresses, and tracks barriers to patient throughput.

　　KNOWLEDGE, SKILLS, AND ABILITIES:

　　Excellent oral and written communication skills.

　　Excellent psychosocial assessment and documentation skills.

　　Basic computer skills required.

　　CPR required within thirty (30) days of hire.

　　Complies with hospital employee health, infection control and safety management programs.

　　QUALIFICATIONS:

　　State licensure (Nevada) - Required

　　Minimum of two (2) years full-time social work experience preferred.

　　Certification in social work or case management preferred

　　Knowledgeable about internal, community and post-acute care resources

　　Basic social worker skills, including but not limited to, empathy, sensitivity to cultural and ethnic diversity, advocacy, communication skills and crisis intervention

　　EDUCATION :

　　Graduate of a school accredited by the Council on Social Work Education.

　　MSW (Master’s Degree in Social Work or equivalent) or BSW with commensurate experience required.

　　LICENSURE:

　　Current State of Nevada Board of Examiners for Social Work license; independently maintains license; Acquires required continuing education units to practice and maintain license.EXPERIENCE:
