JOB REQUIREMENTS: Overview Oncology department Social Worker PRN/Pool -works on an as needed basis Monday - Friday / No weekends or holidaysThe Social Worker BSW coordinates and facilitates patient careactivities to improve continuity of care across the health carecontinuum, to promote appropriate utilization of resources, and tocontribute to improved patient satisfaction and outcomes. The BSWprovides services and support to patients and families who are referredto Social Services or by case finding. The BS provides dischargeplanning as needed. The BSW is a strong patient advocate. Working incollaboration with the Patient Navigator and the Multidisciplinary team,the BSW assesses and intervenes with high risk patients that require ahigh level of continuing care services while ensuring compliance withregulatory standards of care at the organizational, state and federallevel. As needed, the Social Worker conducts clinical review forutilization management; corresponding with payers and utilizingestablished criteria sets. Responsibilities Performs home visits topatientas dwelling. Investigates case situations and presentsinformation to members of the health team on patient\'s family andsocial background pertinent to diagnosis and treatment. Assess patientsfor continuing care needs, creates plan for providing continuing care,and confers with physician and team members as needed. Refers patient orpatient\'s family to other community resources as is indicated; assistspatients and families to complete advance directives. Educates patientsabout resources, options and documents all activities in a timelymanner. The BSW provides emotional support, helps patients and familiesunderstand and accept their condition as well as accepting medicalrecommendations; utilizes community resources to assist patients to livein the community; responsible for travel arrangements for patients;responsible for arranging changes in levels of care and settings; Thehospice BSW participates in IDT meetings and is considered a core memberof the Hospice team per regulations. The home health BSW participates inteam meetings regularly to provide input during patient case conferencesand to educate team members regarding social work interventions that canbe provided. May be required to work flexible hours. Education andExperience Bachelors of Social Work (BSW) from an approved graduateschool of social work Home health or hospice experience desired One yearsocial work experience in a healthcare setting Experience and ability tofacilitate groups Certification/Licensure Currently Licensed (LCSW/LSW)in the State of Illinois and/or Wisconsin Licensed to practice in theState of Wisconsin and/or Illinois or registry eligible Approved CPR orBLS certification required within 90...
