　　JOB SUMMARY & REQUIREMENTS:

　　The LCSW will provide outpatient mental health and co-occurring treatment services to individuals who have a diagnosis of serious mental illness and a co-occurring substance use disorder,

　　The LCSW will provide initial assessment, individual therapy, group therapy, referral and linkage,Design and implements wellness and recovery plans, including individual, group, and/or family psychotherapy, and evaluates and documents patient outcomes according to department policy and procedure,

　　Interprets nature of illness and prescribed course of treatment to patients and families,

　　Communicates family issues to other staff members,

　　Participates in planning conferences,

　　Maintain up-to-date and complete records of patient information according to established standards,

　　As a member of the multidisciplinary team, assist in review

　　Collaborates with other hospital programs and community agencies with which patients and families are involved,

　　Facilitate linkages to outpatient services

　　REQUIREMENTS:

　　Bilingual Spanish Required, LCSW Required,

　　Experience with Co-occurring population a plus
