Position Summary:

　　The social worker will identify resident’s psychosocial, mental and emotional needs and in developing and implementing plans to meet those needs in compliance with facility policies and state and federal regulations.

　　Duties/Responsibilities:

　　• Counsel residents

　　• Conduct cognitive and psychosocial well beings assessments

　　• Discharge planning

　　• Create and oversee discharge summary completion

　　• Create care plans

　　• Make referrals to home care agencies

　　• Schedule appointments

　　• Arrange transportation for residents

　　• Assist with the completion of advanced directives

　　• Conduct interdisciplinary meetings

　　• Advocate for residents and ensure dignity is intact

　　Requirements/Qualifications:

　　• Bachelor’s Degree in Social Work or a related field

　　• Energetic, out-going personality

　　• Ability to work in a fast-paced environment

　　• Team player

　　• Ability to manage multiple tasks and meet deadlines

　　• Previous experience is preferred.
