Providessocial services assessment and interventions for illness-relatedsituations for patients and their families across the care continuum.Other duties as assigned. EXPERIENCE PREFERRED: A minimum of 5 years ofexperience in acute care setting is preferred. EDUCATION DESCRIPTION:Bachelor\'s degree in Social Work is required. SPECIAL SKILLSDESCRIPTION: Analytical skills necessary to assess patients needs,develop associated discharge planning and provide sound advice andguidance. LICENSURE DESCRIPTION: Current State of Wisconsin license insocial work is required.
