　　To ensure all daily living needs and essential services are provided to each youth. This involves creating and supporting a physically and emotionally safe environment for residents and staff as well as utilizing a Therapeutic Crisis Intervention in all program components and interactions.

　　Responsibilities

　　The Social Worker is directly responsible for the provision and coordination of casework services for resident of the Runaway Homeless Youth Shelter TIL Program.

　　Provide casework services to residents of the shelter including referrals for entitlements, housing, and services, accompanying residents on appointments and fostering/facilitating relationships with other services providers to meet the needs of the residents of the program.

　　Develop, in concert with youth and the treatment team, Individualized Service Plans for youth as per DYCD Regulation

　　Develop discharge plans and support networks for residents in the program.

　　Provide individual and group counseling in the facility.

　　Coordinate and monitor multi-disciplinary service provision including facilitating planning conferences and developing service/treatment plans.

　　Ensure the safety of all youth on caseload

　　Maintain daily communication with childcare staff and Program Administrator.

　　Ensure that all residents are in possession of necessary legal documents, i.e., birth certificate, social security card, NY State ID.

　　Assist residents with Independent Living preparation, employment, and educational services.

　　Maintain up-to-date case record documentation and reports including progress notes, court reports, and all documentation required by city and state governing bodies.

　　Provide timely communication to DYCD

　　Participate in staff meetings, multi-disciplinary meetings, and planning meetings

　　Other duties as assigned by supervisor

　　Qualifications

　　Qualifications:

　　MSW/Master's Degree

　　Excellent writing skills.

　　Computer, Word/Excel/Microsoft

　　Valid Driver's License

　　Salary Range $60,000 - $65,000 per year
