Rocky Mountain Cancer Centers, Colorado's largest and most comprehensiveprovider of cancer care has an exciting opportunity for an experienced SocialWorker in Thornton. This is a full-time 36 hours/week position.

Responsibilities
-Meets with patients and their families for psychosocial needs assessment andexamines social support, appropriate community resources, and copingmechanisms.
-Documents all communication accurately and appropriately.
-Provides psychological support to patients and families. Provides support toclinic patients and their families during hospitalizations.
-Provides follow-up counseling on an ongoing basis. Acts as a patient/familyadvocate in negotiating the health care system.
-Develops and maintains support groups to enhance the quality of care topatients and their families. Provides information to patients and familiesregarding resources both in local communities and those geographicallydistant, i.e. those patients contemplating procedures in other cities,BMT, etc.
-Facilitates team support of identified patient and family needs throughongoing communication with clinic team members.
-Provides education to patients and clinic staff regarding skills andknowledge specific to the role of a mental health worker in a medicalpractice. Develops and maintains working relationships with communityagencies, health care organizations, and other providers to promote abroad base of support for patients and families.
-Coordinates support groups with local agencies and health care providers toprevent duplication of services and to better address the needs of the patientpopulation.
-Provides speaking services to groups and organizations to facilitate publicunderstanding of the emotional component of the cancer experience. Facilitatesemployee support groups as needed.
-Demonstrates knowledge of age-specific differences in the patient populationthat include late adolescence, adulthood, middle age, and the elderly.This includes physiological, psychological, and developmental differencesunique to each group.
-Participates in quality improvement activities. May oversee the activities ofstaff providing coordination of Social Services.
-If and only as needed, performs the following duties: completes patientdisability forms, maintains indigent patient drug program, and maintainsknowledge of insurance reimbursement is ues.

Qualifications
Master's degree in Social Work from a school or program accredited by theCouncil on Social Work Education (CSWE). Minimum three- to five-years'experience preferred, working as a masters-prepared social worker in a
