POSITION PURPOSE: The primary purpose of this position is to provide case management services to children with severe emotional disturbance. This position will also complete interviews, assessments, report/petition completion and court attendance/testimony as well as provide community outreach for mental health services and support families in crisis while adhering to the mission of Morrison County Social Services. Duties include, but are not limited to, determining eligibility, assessment of strengths and needs and functionality, identifying, planning, arranging, coordinating and monitoring services all in accordance with applicable Rule, Statute, any other federal or state regulations and Agency policy. PRINCIPLE RESPONSIBILITIES: Determine Client eligibility for Children's Mental Health Services within the guidelines of Minnesota Statute and Rule. Provide Case Management Services within guidelines of Minnesota Statue for Children's Mental Health, Child Welfare, and Waiver Case Management. Complete weekly time reporting and provides written documentation for case management support with a goal of 70% workgroup focused time allocation. Provides support to and monitors foster placements. Provides arrangement of placement, monitoring of service needs, and court recommendations for AMH clients involved with Commitment services. Develop and maintain positive working relationships within and outside the Agency. Conducts practice and reporting in a method that supports revenue enhancement for the agency or client. Assist in establishing Agency resources for prevention and treatment of mental health issues on an ongoing basis. Maintains Profession Standards and Follows Legal Guidelines Provide quality customer service to internal and external customers. Complete other duties as assigned by the Agency. Knowledge of social casework objectives, principles, and methods. Knowledge of social welfare research methods. Knowledge of family systems and dynamics. Ability to effectively apply casework knowledge and skills. Ability to develop and maintain effective working relationships with clients, relatives, the courts, local law enforcement agencies, other community service agencies, and the public.
