Job Description
The Iowa Department of Health and Human Services (HHS) is seeking a Social Worker 2 to join the team at the Glenwood Resource Center (GRC). This is a full-time position with a Monday-Friday, 8 a.m.-4:30p.m. schedule. Some evening and weekend work and a small amount of travel might be required.
Employees are eligible for a $1,500 retention payment for a commitment of 7 pay periods (14 weeks) and are eligible for renewal at the end of each 7 pay periods until the facility closes. Speak to the hiring manager for more eligibility information.
CMS regulations require that the person hired for this position must have at least a bachelor’s degree in a human services field (including, but not limited to sociology, special education, rehabilitation counseling, and psychology). Only applicants who meet these requirements will be considered for the position.
This Social Worker 2 supports individuals and their families as they are provided services at and transition from GRC. This position functions as the primary liaison with individuals in care, families/guardians, community providers, case managers, regional/county mental health and disabilities staff, Managed Care Organizations (MCOs), internal teams, and others invested in outcomes for individuals supported at GRC. The Social Worker 2 is an advocate for the individual and their family/guardians within the interdisciplinary team (IDT) and must establish and maintain effective working relationships and open dialogues, promote inclusion, foster participation of individuals in treatment planning, and maintain close communication with guardians/families about participation opportunities, significant updates, and changes.
This position supports successful transitions to the community by engaging the individual, supporters, IDT members, the MCO case manager, and others as appropriate in education and discussion about supports and services available within the state. The Social Worker 2 assists with identifying barriers to community-based living and identifies and discusses skills the individual will need to successfully transition. Full support of a person-centered approach to treatment planning with a focus on consistently working toward successful transition is key to success in this position.

Minimum Qualification Requirements
Graduation from an accredited four-year college or university;
OR
the equivalent of four years of full-time technical work experience involving direct contact with people in overcoming their social, economic, psychological, or health problems;
OR
an equivalent combination of education and experience substituting the equivalent of one year of full-time qualifying work experience for one year (thirty semester or equivalent hours) of the required education to a maximum substitution of four years.

Additional Qualification Requirements
Designated positions in this job class require applicants to obtain the required Chauffeurs License and endorsements within a period of time as determined by the appointing authority at the time of hire.
