We are currently seeking a skilled visit RN to provide care in the Red Wing community and surrounding areas. This is a part-time position (24-30 hours per week), with the potential to increase to full-time. Day shift, NO holidays, NO weekends, flexible schedule! Requirements: - Must have an active RN license in MN, with an interest in being licensed in Wisconsin. -Must have a valid driver's license. Pay: $36.00 - $40.25 per hour (DOE) Core Responsibilities: - Provides skilled nursing visits to clients in their homes. Includes wound care, med set up, IV's, and more for both pediatrics adults. - Confers with other members of the client care team to ensure quality patient care. - Completes documentation of in-home admissions, assessments, and visits. - Develops and implements an individualized client plan of care. - Completes home care certification, recertification, and OASIS forms. - Reports progress and changes in client condition to physicians, DOCS, and referral sources. - Evaluates effectiveness of interventions and progress toward client goals.
