　　The role you’ll contribute:

　　Under general supervision, serves as supervisor of clinical and clerical support staff on an assigned shift. Responsible for coordinating direct admissions and hospital transfers to an appropriate Advent Health Hospital. You will apply your nursing knowledge derived from education and experience as foundation for gathering appropriate clinical data to ensure appropriate placement, transfer facilitation, communication and accurate documentation.

　　The value you’ll bring to the team:

　　You will coordinate ambulance transfers from the Advent Health Hospitals to other facilities and post-acute care providers.

　　You will work in a customer-centric transfer and transport call-center; respond to all callers with the aid of service profiles and healthcare information.

　　You will consult with on-call physicians regarding patient care issues, needs, and concerns and advise when appropriate.

　　You will work collaboratively with AH facilities to manage all external patient movements. You will facilitate inner facility transfers, maintaining communication with parties, including but not limited to attending/accepting MD and referring facility.

　　You will communicate with physicians and outlying facilities if placement is problematic.

　　You will be responsible for documenting and tracking all hospital transfers and transports

　　You will exhibit excellent customer relationship skills including telephone and personal contact with the call centers customer groups (physicians, nurses, vendors, fellow employees).

　　You will establish and maintain a high degree of awareness and knowledge of participating AH medical services.

　　Proficient in EMTALA.

　　Qualifications

　　The expertise and experiences you’ll need to succeed:

　　Minimum qualifications :

　　BSN

　　5+ years of nursing experience

　　BLS from American Heart Association

　　Preferred qualifications:

　　MSN

　　2+ years of experience in ED/ICU and/or House Supervisor role
