Position Summary: Providesprescribed medical treatment, medication and personal care services tonursing home residents to meet individual spiritual, physical, emotionaland social needs consistent with the plan of care, Standard Nursingpractice, scope of gerontological nursing procedures and all applicableregulations, codes, policies and procedures. Essential Duties andResponsibilities: This list of duties and responsibilities is notall-inclusive and may be expanded to include other duties andresponsibilities, as management may deem necessary from time to time.Receives verbal report and communicates all pertinent resident careinformation to the CNAs on assigned unit or area. Visits each residentto assure the appropriate cares are being provided, reports any problemsto the manager/supervisor. Administers and documents medication andtreatments, etc. within the parameters of the LPN licensure andaccording to the care plan. Reports any changes to themanager/supervisor. Reports significant changes in residents\'conditions immediately to the manager/supervisor and notifiesappropriate people as directed Maintains timely, accurate writtencharts, documents, records as required by law and/or regulation policiesand procedures. Charting must be informative, descriptive or nursingservices provided, residents\' responses to cares and a notation whenfamilies/physicians are notified of significant resident changes. Reviewand revised care plans as changes occur to reflect the exact conditionof each resident Completes compliance rounds or resident rooms to assurea clean, orderly, comfortable environment. Makes adjustments based onindividual needs. Appropriately identifies problems, approaches andgoals on residents\' care plans and discusses them with the CNAs dailyafter compliance rounds. Writes timely nursing summaries that reflectresidents\' conditions, cares being given, restorative/rehabilitationinterventions, the movement toward appropriate goals and acceptableapproaches. Participates in resident assessment on admission, on-goingresident care/family care conferences and discharge planning as requiredMakes rounds with Medical Director, attending physicians, nurse manager,DON as delegates Assures that all resident rights are maintained at alltimes.
