Purpose of Job This position is responsible for accepting referrals from appropriate entities, verifying benefit information, and acquiring initial authorization. In addition, this position is responsible for eliciting all pertinent clinical and social data required to accurately determine need for services. Job Functions Assures that all referral sources and the general public have fast, easy access to RHHS. Accurately documents and communicates patient needs. Verifies insurance information and appropriate sequencing of payer. Screens referrals for reimbursement, primary clinics, and initial authorizations. Clearly communicates Home Care, Hospice, the Marie Steiner Kelting Hospice home, and Home Support services. Acts as a liaison between staff and physicians for accurate MD orders, lab results, questions, and/or messages as requested. Performs other duties as assigned. Minimum Education/Work Experience Graduate of an accredited school of practical nursing Experience with computer database, work processing, and spreadsheet programs Knowledge/Skills/Abilities Strong communication and interpersonal skills Ability to meet and maintain the necessary background checks as aligned with position functions Ability to communicate in the English language for effective written and verbal correspondence in order to complete job functions as mentioned above Capable of making independent decisions with the use of good critical thinking and problem solving skills Ability to work autonomously and as a team Ability to communicate in the English language for effective written and verbal correspondence in order to complete job functions as mentioned above License/Certifications Current Practical Nurse license in Minnesota or if licensed in another state, or a permit from the Minnesota Board of Nursing while the license application is in process Valid MN Driver's license and meets Ridgeview's age and insurability requirements. Preferred Qualifications Three years home care or hospice experience This position will cross train to hospice and home health visit nurse.
