  Description:

  Currently offering up to $10,000 in recruitment bonuses for qualified energetic RN’s who are passionate about the profession. We strive for flexible scheduling and work hard to balance and meet the needs of the department and staff. Minimal weekend or holiday rotations.

  In collaboration with the other professionals assist with development and implementation of the discharge plan of care. Supports the discharge plan by coordinating and scheduling follow up care and appointments with patient and support team. Interacts with the Physician, patients and family as needed. Serves as a resource for the care team through support, education, and clinical expertise.

  What you will do:

  Attend SNAP huddles if needed and works collaboratively with unit staff and support team (RN, Physician/Clinical Affiliate, DPA, MSW, CDI, Rehab Services, etc.) to progress each patient's care towards discharge.

  Serves as a consultant and educator with medical staff regarding status, placement, payer requirements, and utilization of resources.

  Schedule’s care appointments, documents and educates care team for appropriate follow up care.

  Reviews LOS values and working DRG daily; Updates in Care Logistics software as needed.

  Identifies opportunities to transition testing to the outpatient setting and escalates as appropriate

  Identifies/assesses patient for readmission risk, communicates to care team, and updates Care Logistics software.

  Precept and Mentor students and colleagues.

  Responsibilities:

  Ensures quality care is rendered to all patients in accordance with the New York Regulatory Agencies, National Standards of Practice, and Nursing and Clinical Service standards of care and practice.

  Maintains current knowledge of basic health care economics, trends and reimbursements methodologies and applies knowledge to daily practice. Recognizes situations that require immediate intervention. Continues to develop professionally and apply knowledge and skills necessary to provide care appropriate to each patient.

  Involved in preparing the discharge plan for patients and families.

  Education, Training, Experience, Certification and Licensure:

  AAS degree in appropriate clinical discipline with one to two years progressive experience in the field.

  Bachelor’s degree in appropriate discipline depending on specific service needs and/or requirements.

  NYS Registered Nurse licensure or clinical licensure in appropriate field as required by regulatory agency.

  Minimum of 2 years clinical experience in a hospital setting, preferred.

  Previous discharge planning, home care, and/or managed care experience desired.

  Participates in orientation, education conference, updates and maintains knowledge and skills related to specific areas of practice.

  National certification encouraged and supported.

  Organizational, management and leadership skills required.

  Current certification in BLS/CPR.

  Competent in oral and written communication skills.

  Established aptitude for problem solving and conflict management.

  Work Environment and Hazards:

  OSHA category I. Routine or potential exposure to blood, body fluids, excretion or secretions.Physical Demands:

  Moderate work: lifting, bending, stooping, pushing and pulling. Heavy work: standing and walking.Work Contact Group:

  All services medical staff, patients, visitors, contractors, vendors, and various regulatory and professional agencies.Supervised By: Manager, Department Coordinator and Director

  Pay Range: $34.05 - $51.10

  Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
