About RN (Case Manager)

　　Case Manager (RN) - Salary range from \$41.00 to \$48.00/hour Full Time40 hours Monday - Friday Primary Responsibilities: Coordinates andoversees each patient\'s care from admission through post?-dischargefollow up. Maintains communication between the interdisciplinary team,patient/family, and external case managers from payors and otherfacilities. Prerequisites: Minimum of five (5) years health careexperience, registered nurse preferred with a specialty certification.Effective verbal and written communication skills. Previous UtilizationReview and/or Case Management experience or training. Knowledge ofmanaged care, Medicare, and federal, state, and local regulations.Knowledge of growth and development across the life span required.
