This position is located at 3475 N Saratoga St, Oak Harbor, WA 98277.The case manager will be responsible for coordination of care for active-military members and their families/dependants.This is a Monday-Friday position with no required overtime.Hours: 8 hour shifts between 0730-1630 (7:30AM - 4:30PM) 1HOUR LunchCompetitive Compensation, with an estimated annual income of $93,000Description/Job Duties:Knowledge and skills to effectively apply the following core case management functions:Assessment - ID patients for case management; comprehensive collection of patient information and medical status; and continue evaluation of an established plan of carePlanning - Collaboration with the patient, family/caregiver, primary provider and other members of the health care team for developing an effective plan of careFacilitation - Care coordination and communication among all involved partiesAdvocacy - Support for the patient and family/caregivers to ensure identified education and appropriate, timely care is received.Excellent communication skillsWorking with physically and/or mentally ill patients who may be uncooperative, fearful, emotionally distraught, and occasionally dangerous.Organization, problem solving, and communication skills required to articulate medical requirements to patients, family/caregiver, medical and non-medical staff in a professional and courteous way.This role will require clinical case management experience in working with families, adults, children, seniors and groups.REQUIREMENTS:ASSOCIATES DEGREE WITH 3 YEARS OF EXPERIENCE or BACHELORS DEGREE AND 2 YEARS OF EXPERIENCE.WA NURSING LICENSECandidates should be familiar with Microsoft Office programs, MS Outlook (e-mail), and HIPPA guidelines.This role is for a medical professional on a military base.
