RN Care Manager -Sunnyview Acute Rehab Hospital -Per Diem

What you will do:
Provide discharge planning and case management services to inpatients on assigned unit/program.
Attend and participate in departmental meetings and in-services
Participate in any designated hospital committees as assigned by Department Manager.
What you will need:
Associates in Nursing and NYS RN License Required
BS in Nursing or clinical practice preferred
Community Health experience and/or CRRN recommended
Strong organizational skills, verbal and written communication, assessment, and problem solving skills are essential.
Must have ability to interact positively with patients, families, community agencies, and providers
Pay Range: $31.50-$44.95
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
