JOB REQUIREMENTS: Job Description and Essential Functions JobTitle/Position: RN Care Manager Job Classification: Direct/IndirectPatient Care Date of Analysis: Date of Revision/Review: 08/2018__________________________________________________________________________________________Brief Description of Job Responsibilities: Responsible for coordinatingthe continuum of care for assigned patients and ensure optimumutilization of resources, excellent service delivery and positivequality outcomes, while minimizing unnecessary medical expenses.Specialized Responsibilities: (This list may not include all of theduties assigned) * Implements evidence based treatment guidelines ofclinically diagnosed condition. * Prepare chart of assigned patientsprior to appointment. * Follow protocols for standing orders formedication refills, ordering tests and delivery of routine preventativeservices. * Educate patients and families about managing theircondition and or disease. * Conduct pre-visit planning with providers.* Write individual care plan with treatment goals. * Assess patientbarriers to unsuccessful outcomes as it relates to medication,appointments and goals. * Coordinate visits with multiple cliniciansand or diagnostic tests during one visit. * Determine through triagehow soon patients need to be seen. * Provide telephonic advice onclinic issues. * Identify health insurance or financial resources forpatients. * Systematically send or communicate medical information topatients. * Review clinical measurements with patients at visits. *Review information from facilities (discharge summary, hospitaladmission or ER visits) to determine if patients require a follow upvisit or who are at risk for adverse outcomes. * Contact patients afterdischarge from facilities. * Provide or coordinate follow up care topatients/families who have been discharged. * Develop writtentransitionon plan in collaboration with family and patients whentransitioning to another facility or provider. * Aid in identifying anew primary care physician or specialist or consultant and offer ongoingconsultation.Performance Requirements: *Education/Experience/Certifications: * Excellent communication, writtenand analytical skills. * * RN with current Wisconsin license. *Minimum of three years in clinic setting. * Prefer specialty or primarycare with EMR experience. * Excellent assessment skills.Non-EssentialDuties: Combination of medical office and exam/procedure room settings.Possible exposure to communicable diseases, toxic substances, medicinalpreparations and other conditions common to a clinic environment.Typical Physical Demands: * * * Typical Working Conditions: * Otherduties as assigned. * Requires full range of body motion includinghandling and lifting patients, manual...
