About the Position

Plans, implements, and evaluates designated MDS assessments in conjunction with the Care Plan Team ensuring quality of care areas are identified. Essential functions include but are not limited to:

Administrative

Provides leadership to interdisciplinary team (IDT) for management of the MDS, Clinical Reimbursement, Care Plan processes and utilization management processes in accordance with current federal, state, and local requirements and regulations and company policies and procedures.

Provides direct supervision to other MDS Coordinators and ensures their work is in compliance with State and Federal regulations and Goodwin Living policies and procedures.

Provides leadership in community for financial implications of RUGS, PDPM, case mix index rates.

Minimum Data Set (MDS)

Coordinates the Resident Assessment Instrument (RAI) process. Leads and directs the completion of the MDS ensuring compliance with all federal and state regulations related to MDS assessments.

Ensures that the MDS/RAI deadlines and accuracy requirements are met while ensuring that coding most appropriately captures each resident's acuity for accurate and optimal reimbursement.

Monitoring and management of skilled services provided.

Other Responsibilities

Manages the UR process, to include tracking of skilled days and stay.

Works with the Finance Team to address ADRs.

Collaborates with the Social Worker to manage the paperwork for Medicaid residents.

Monitors the Quality Measure and 5-Star Rating, makes recommendations, and implements plans to address concerns.

Reviews charts periodically to check medications based on the Polypharmacy Policy.

Qualifications

Associate’s degree required; Bachelor’s Degree preferred.

Current RN license in good standing in VA.

In-depth understanding of federal and state regulations related to MDS assessments.

Extensive MDS 3.0 experience needed. 3+ years as an MDS/PPS/PDPM Coordinator.

RAC-CT certification (or acceptable equivalent) required within one year of employment if not certified at the time of hire. Must maintain certification as current once certified.

In-depth experience and knowledge with PDPM and CMI related to Medicare and Medicaid service reimbursements.

A minimum of one-year experience in Gerontological Nursing required, preferably obtained in a long-term facility. Strong clinical assessment skills.

Proficiency in electronic health records (EHR) systems and MDS software.

Able to maintain a flexible work schedule in accord with the needs of the facility.

CPR certification.
