Primary Function/General Purpose of Position
The Heart and Vascular Service Line Coordinator provides operational support and project coordination to market medical directors and service line leadership. Responsibilities include planning, organizing, implementing, and operationalizing key initiatives of the service line across multiple sites of care in the Richmond Market.
Essential Job Functions
Collects and analyzes operational, quality, and financial data to populate service line dashboards and scorecards supporting the work of the service line and clinical subcommittees
Provides project management support to market wide initiatives promoting quality, growth, access to care and market wide standardization of care delivery processes
Provides administrative support to the service line council and subcommittees including preparing presentations and materials, scheduling, and taking minutes
Develops communication and education to coordinate care across teams and locations
Works to connect and coordinate service line work by working closely with nursing leadership and medical group operations
Licensing/Certification
Current RN license in state where they are working or covered by compact (required)
Education
Associate of Science Nursing (required)
Graduate degree in Heath Administration, Public Health, Business Administration or other related field (preferred)
Work Experience
2 years of recent experience in an acute care setting (required)
2 years of experience in coordinating and or managing improvement initiatives involving multiple stakeholders within and across facilities (preferred)

EPIC Electronic Health Record (preferred)

Demonstrates knowledge of the principles of growth and development of the life span and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his or her age, specific needs and to provide the care needed as described in departmental policies and procedures.
