Liberty Home Care is looking for a motivated Registered Nurse to join our growing home care nursing team.

　　Duties & Responsibilities

　　Provide skilled nursing services to agency patients.

　　Provide assessment of newly referred patients, assess health needs and eligibility for home care.

　　Provide patient and family teaching prior to discharge and on an on-going basis.

　　Develop and implement care plans in conjunction with patient’s authorized medical practitioner.

　　Reassess patient needs for services on a continual basis and discuss any changes in the plan of treatment with the patient physician/family.

　　Prepare written documentation and summaries of the patient’s progress as needed for the continuation of home health services.

　　Plan, facilitate, and document patient’s discharge.

　　Provide supervision and training of PCA/HHA staff

　　Assist with the on-call rotation.

　　Qualifications

　　NYS Licensed Registered Professional Nurse.

　　Minimum of Associate's Degree required.

　　One year Nursing experience required.

　　One year home care experience a plus.
