About the Team
The Assertive Community Treatment (ACT) team is community based and supports some of the community's most acute individuals with chronic mental illness and substance use in Anchorage, Alaska. The work is fast-paced, unpredictable, and requires risk management within the ACT model. The ACT Team provides services to previously homeless individuals suffering from mental illness or a substance use disorder.
What Youll Do
Maintain a caseload and work with other members of the ACT Team to promote stability, recovery, and community integrationMonitor vital signs, glucose, medication side effects, etc. and report findings as requiredAdminister and provide medication to patients according to physician/ANP/PA ordersWork with team and patient in developing/following the optimal treatment/service planParticipate in facilitating crisis interventions, referrals, and hospitalizations as appropriateGood To Know
