  Pod RN duties include:

  In basket oversight of messages and needs within the electronic medical record Regulatory and Quality management activities Partner with Practice Manager and Lead Physician to improve the delivery of nursing care at the site level based on individual practice needs Collaborate with Nursing Education team to ensure a successful onboarding for new clinical support team members Dissemination of information to clinical practice teams Clinical resource by licensure Assess walk in patients that require onsite triage

  Pod RNs function across one or more practices, based on patient panel size and geography. Depending on the number of practice sites included in the identified pod, the nurse will rotate her daily schedule between her pod practices with the goal of being onsite at least once per week at each site. They are physically located at one site per day but expected to support all practices every day.

  As a POD nurse, you are key in the success of operations and compassionate patient care every day.

  STATUS: Full Time

  LOCATION: Riedman Health Center

  DEPARTMENT: Internal Medicine

  SCHEDULE: Monday-Friday, Days

  RESPONSIBILITIES:

  Patient Care & Service. Promote and restore patients' health by completing the nursing process; collaborating with physicians and multidisciplinary team members; performing various treatment procedures; providing physical, educational and emotional support to patients, friends and families; supervising assigned team members

  Planning & Communication. Develop and document individualized care plans customized for each patient's unique needs, with support from the interdisciplinary health team as needed; maintain effective communication to convey patient health status, treatment plans and progress

  Electronic Health Record (EHR) Management. Demonstrate proficient use of an EHR - including accurate patient and provider documentation and communication

  Compliance. Adhere to required department and system protocols, regulations (local, state, federal) and education requirements

  EDUCATION:

  AS: Nursing (Required)PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

  LICENSES/CERTIFICATIONS:

  BLS - Basic Life Support - American Heart Association (AHA)RN - Registered Nurse - New York State Education Department (NYSED)PAY RANGE: $32.33 - $45.00

  The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
