　　Job Summary and Qualifications

　　As a Palliative Care Nurse Coordinator, you will be responsible for the development and implementation of a palliative care program. You will provide guidance, support, and education to healthcare team members in the overall delivery of services related to palliative care.

　　You will establish a palliative care plan for all palliative care patients based upon assessments, patient care standards, and patient and family preferences

　　You will consult and collaborate with healthcare team members to develop a plan of care and palliative care recommendations

　　You will ensure consistent, complete, and thorough nursing documentation

　　You will review physician orders related to palliative care, and write or enter orders into the system accurately

　　You will provide timely communication to nurses and/or physicians regarding changes to the plan of care and/or changes in the patient’s condition

　　If necessary, you will provide basic life support in collaboration with the healthcare team

　　You will maintain and review patient records, charts, and documents for accuracy, quality, and best practices related to palliative care

　　You will provide palliative care data, including the number of consults, referrals, conversions to hospice, and changes in level of care

　　Job Requirements

　　Experience

　　1 year of clinical nursing experience as a Registered Nurse (RN)

　　Previous experience in Hospice and Palliative Care nursing

　　Critical care experience in a hospital setting – preferred

　　Education

　　Associate’s degree in Nursing (ADN)

　　Bachelor of Science in Nursing (BSN) – preferred

　　Advanced Practice Nurse (APN) – preferred

　　Licenses and Certifications

　　Current State of Texas Registered Nurse (RN) license or Compact Registered Nursing License. If Compact License, Texas license must be obtained within 90 days of hire

　　Basic Life Support (BLS) certification as per St. David’s Healthcare policy

　　Certified Palliative and Hospice Care Nurse (CHPN) – preferred
